These meetings are conducted on a monthly basis and are focused on understanding the factors that might have contributed to the morbidities and mortalities registered in the previous month. Discussions on how to improve and minimize the re-occurrence of the same are conducted with a clear way forward plan laid out. Reflection on the previous meetings’ way forward plan is also made to assess the progress. Each meeting goes for a duration of 3 hours or even longer depending on the matters arising. A total number of 10 meetings have been conducted, which amounts to 30 hours.
