| Details | Presenter: Medical team, These meetings are conducted on a regular basis and are focused on understanding the factors that might have contributed to the morbidities and mortalities registered in the previous month. Discussions on how to improve and minimize the re-occurrence of the same are conducted with a clear way forward plan laid out. Reflection on the previous meetings’ way forward plan is also made to assess the progress. Among the morbidities include; surgical related complications both immediate and long term, Hospital acquired infections and the preventive measures that are in place that is ; hand hygiene and critical care bundles. Also different clinical case scenarios are discussed in such meetings to identify the gaps & near misses. Each meeting goes for a duration of 2 hours or even longer depending on the matters arising. A total number of 6 meetings have been conducted, which amounts to 12 hours. |
